After achieving sustained graft functioning in animal studies, the next step in the progression of xenotransplantation towards clinical application is the initiation of the first clinical trials. This raises the question according to which criteria patients shall be selected for these trials. While the discussion regarding medical criteria has already commenced, ethical considerations must also be taken into account. This is essential, first, to establish a procedure that is ethically reasonable and justified. Second, it is a prerequisite for a publicly (...) acceptable and comprehensible implementation. This paper outlines a multicriteria approach for the selection of patients in first-in-human clinical trials of cardiac xenotransplantation with four ethical criteria: medical need, capacity to benefit, patient choice and compliance (as an exclusion criterion). Consequently, these criteria identify a primary target group of patients who exhibit a high medical need for cardiac xenotransplantation, face a high risk of morbidity and mortality without an organ replcaement therapy, have a substantial chance of benefiting from xenotransplantation, thereby also enhancing the scientific value of the trial, and qualify for an allotransplant to have a real choice between participating in a first-in-human xenotransplantation trial and waiting for a human organ. A secondary group would include patients for whom only the first two criteria are met, that is, who have a high medical need and a good capacity to benefit from xenotransplantation but who have a restricted choice because they do not qualify for an allotransplant. ZusammenfassungBasierend auf einer systematischen Literaturrecherche über den Sport im Strafvollzug geht das Review der Frage nach, wie die Inhaftierten den Gefängnissport erfahren und reflektieren. Gestützt auf zehn qualitative Studien werden die zentralen Bedeutungsfacetten des Sports in der Haft herausgearbeitet. Grundsätzlich wird darauf verwiesen, dass der Sport für die Häftlinge ein wichtiges Medium bei der Bewältigung der mit der Inhaftierung typischerweise einhergehenden negativen Begleiterscheinungen ist: Die Teilnahme am Sport ermöglicht es ihnen, die Monotonie und Langeweile des Gefängnisalltags aufzubrechen, Stress abzubauen und (...) das Gefühl von sozialer Isolation zu reduzieren. Darüber hinaus bietet der Gefängnissport in einer totalitären Institution eine der wenigen Nischen, in denen sich die (männlichen) Gefangenen temporär als selbstbestimmte Subjekte erfahren können. Negative Einflüsse des Sports werden in bisherigen Studien kaum thematisiert, ebenso wenig wird Sport als Mittel der Resozialisierung gesehen. Vor diesem Hintergrund werden Forschungslücken und Perspektiven für zukünftige Studien aufgezeigt. (shrink)

The goal is to present how shared decision-making in paediatric oncology occurs from the viewpoints of parents and physicians. Eight Swiss Pediatric Oncology Group centres participated in this prospective study. The sample comprised a parent and physician of the minor patient. Surveys were statistically analysed by comparing physicians’ and parents’ perspectives and by evaluating factors associated with children’s actual involvement. Perspectives of ninety-one parents and twenty physicians were obtained for 151 children. Results indicate that for six aspects of information provision (...) examined, parents’ and physicians’ perceptions differed. Moreover, parents felt that the children were more competent to understand diagnosis and prognosis, assessed the disease of the children as worse, and reported higher satisfaction with decision-making on the part of the children. A patient’s age and gender predicted involvement. Older children and girls were more likely to be involved. In the decision-making process, parents held a less active role than they actually wanted. Physicians should take measures to ensure that provided information is understood correctly. Furthermore, they should work towards creating awareness for systematic differences between parents and physicians with respect to the perception of the child, the disease, and shared decision-making. In a speech at the Independence Hall in Philadelphia (Pennsylvania) held on 4th of July when he was awarded with the „Peace Medal”, President Vaclav Havel has exemplified the meaning of the postmodern age as follows: a Bedouin, sitting on a camel, wearing jeans under his traditional clothes, drinking Coke and listening to a walkman with a Coca-Cola ad stuck onto the camel. True indeed. Closed cultures are breaking up and become westernized, and along with this development many ecological and (...) social problems have occurred. From the standpoint of the given example, we could ask, where did the Bedouin get all this? Where can he recycle his empty tins? Let us discuss these social and ecological difficulties. (shrink)

John Rawls ist im 20. Jahrhundert sicherlich einer der bedeutendsten praktischen Philosophen. Seine Werke und die damit verbundene Begriffs- und Theoriebildung haben viele Traditionslinien philosophischer Klassiker der Moderne aufgegriffen, synthetisiert und neu ausgelegt. Mit seinem Nachdenken über Gerechtigkeit prägt er bis heute viele philosophische Debatten, sei es in positiver Bezugnahme oder negativer Abgrenzung.

Schleiermachers Wissenschaftslehre wird in dieser Untersuchung als eine universale Prinzipienlehre und formale Strukturtheorie rekonstruiert. This paper discerns two types of mathematization, a foundational and an explorative one. The foundational perspective is well-established, but we argue that the explorative type is essential when approaching the problem of applicability and how it influences our conception of mathematics. The first part of the paper argues that a philosophical transformation made explorative mathematization possible. This transformation took place in early modernity when sense acquired partial independence from reference. The second part of the paper discusses a series of examples (...) from the history of mathematics that highlight the complementary nature of the foundational and exploratory types of mathematization. Singh proposes a cultural evolutionary theory of shamanic practices, including trance. We argue that cultural factors are deeply intertwined with biological aspects in shaping shamanic practices, and the underlying biology is critical. We discuss the neural underpinnings of rhythm-induced trance, how they can facilitate insight, and how altered states can promote healing.

Diversity and hybridity, interaction and exchange: these terms compose the thematic focus of this issue of the journal “Der Islam”: Eastern Christianity in History and the Present: Perspectives and Obstacles. The issue features contributions to a conference held in the year 2008. Evaluating the state of research on Eastern Christianity in Germany, the participants observed that this field of academic enquiry is in danger of becoming extinct in this country. Thus, this collection of articles hopefully serves as a catalyst for (...) jumpstarting debate on Eastern Christianity in the German academic community. The study of Eastern Christian communities’ cultural identity is at the same time ineluctably intertwined with Qur‘anic Studies and Islamic History - i. e. an Islam in dialogue with, but not mimicking Jewish and Christian traditions of the Hijaz. Diversity in the Islamic-ruled world is a historical reality that has been largely ignored in the study of Islam as well. This edition of “Der Islam” therefore highlights research that considers the interaction between Muslim and non-Muslim groups, as well as the entangled development of the religious traditions, in order to better understand the socio-religious plurality of Western Asia. Interaction and exchange, however, also include moments of conflict and discrimination. As we close upon the one-year anniversary of the “Arab Spring”, the authors are reminded that it is important to consider the worsening condition of Eastern Christian communities today and their representation in Western studies as well as in the political systems they live in. The prevalence of nonpaternity in human societies is difficult to establish. To obtain a current and fairly unbiased estimate of the nonpaternity rate in Germany, we analysed a dataset consisting of 971 children and their parents in whom human leukocyte antigen (HLA) typing had been carried out in the context of bone marrow transplantation. In this sample, nine exclusions (0.93%) could be identified on the basis of more than 300 HLA-haplotypes defined by four HLA genes. Given this number of exclusions, (...) a maximum likelihood estimate of the nonpaternity rate in the population of 0.94% was obtained with asymptotic 95% confidence limits of 0.33% and 1.55%, respectively. This result is in accordance with recent surveys as well as findings from Switzerland for a comparable sample, and it suggests that earlier estimates of the nonpaternity rate which were often in excess of 10% may have been largely exaggerated. If by democracy we mean not only a form of govern- ment but also a system of living, then indeed a unanimous judgment and also a general conception cannot be expected, but nevertheless the concept need not to be debased to the stage of complete (...) meaninglessness. In this paper I will deal with some aspects of dialogue, love and multiculturalism. In the first and the second aspect I see very much important presuppositions of a multiculturalism enriching each other. If the person is not able to communicate, to love and define what goals she/he would want to achieve, the person will be not able to enrich the multicultural world! Therefore, I am pressing on the conditions and kinds of dialogue, of the art of love, and in (...) the end, I am giving some ideas about the multiculturalism. (shrink)

The free world stands and falls on its cultural and religious policies, which affect not only the social structures within countries, but also the relations between people and peoples, generations and nations. No culture can exist in the abstract, and therefore no one can take an intelligent interest in cultural and religious affairs without a clear and consistent philosophy of life. However, after years of development we see a widening gap between people and groups in the same society caused by (...) different viewpoints on the same issues. The „fringe population” becomes more and more disillusioned because of unemployment or poverty. In this marginalized zone lie the strongest roots of radical issues, because these people do not have very much to lose. These people find hope and solutions in the radical ideas of extreme groups, i.e. in religious sects, in right-wing quasi-political groups (the Neo-Nazis) or left-wing groups (Neo-Communists). It may prove fruitful to look into the fundamentals of the tension between relativism and radicalism for a better understanding the role of morality and ethics in a globalized world. (shrink)

Der Gegenstand der Medizinethik ist die normative Analyse und ethische Reflexion von praktischen Fragen, denen sich Einzelne oder Gruppen mit Blick auf das Gesundheitswesen und insbesondere mit Blick auf den Umgang mit menschlicher Krankheit und Gesundheit gegenübersehen. Sie gehört damit zu den sogenannten angewandten oder Bereichsethiken.

In diesem Beitrag sollen zentrale Spielarten und Familienverwandte des Liberalismus vorgestellt sowie auf zentrale Herausforderungen eingegangen werden, die sich dem Liberalismus als politischer Theorie gegenwärtig stellen. In diesem Beitrag sollen zum einen zentrale Spielarten und Familienverwandte von Rawls’ Liberalismus vorgestellt werden. Dabei handelt es sich um den Links- und Rechtslibertarismus, den Neoliberalismus sowie den Nationalliberalismus. Zum anderen sollen mit dem gesellschaftlichen Pluralismus, dem Verhältnis zur Demokratie und der Umweltzerstörung wichtige Herausforderungen thematisiert werden, mit denen sich der politische Liberalismus heutzutage konfrontiert (...) sieht. I would like to look at some of the challenges of ethcis today. Therefore, in the first part I say something about ethics, the ethical theories and ethical concepts. Afterwords I am going to explain a little bit about the human dimensions, the dealing with experiences (i.e. work); because the human person has to decide the right thing in the right place on the right time, and in relatively freedom. In the end, there are some ideas about applied ethics which (...) is necessary to focusing on the practical issues, too. In this paper, we examine the use of the term ‘life’ in the debates within and about synthetic biology. We review different positions within these debates, focusing on the historical background, the constructive epistemology of laboratory research and the pros and cons of metaphorical speech. We argue that ‘life’ is used as buzzword, as folk concept, and as theoretical concept in inhomogeneous ways. Extending beyond the review of the significant literature, we also argue that ‘life’ can be understood as aBurstwordin (...) two concrete senses. Firstly, terms such as life easily turn into fuzzy, foggy and buzzy clouds of nonsense, if their use is not appropriately reflected. In these cases, the semantic orientation is detonated. This is theBurstword Icharacteristic of the concept of ‘life’ that we reveal for its unclear terminological use. Secondly, and in contrast toBurstword I, we show that the concept of ‘life’ can be used in a methodologically controlled way. We call this kind of useBurstword II. Here the concept of ‘life’ fulfils the function of expanding an inadequately narrow disciplinary or conceptual focus in different discursive contexts. In this second sense, ‘life’ receives an important operational function, for instance as a transdisciplinary research principle. It turns out that the innovative function and paradigm-changing power of metaphorical speech belong here as well. Finally, we illustrate three ethically relevant examples that show how ‘life’ can be applied asBurstword IIin the context of synthetic biology. (shrink)

We motivate and formalize the idea of sameness by default: two objects are considered the same if they cannot be proved to be different. This idea turns out to be useful for a number of widely different applications, including natural language processing, reasoning with incomplete information, and even philosophical paradoxes. We consider two formalizations of this notion, both of which are based on Reiter’s Default Logic. The first formalization is a new relation of indistinguishability that is introduced by default. We (...) prove that the corresponding default theory has a unique extension, in which every two objects are indistinguishable if and only if their non-equality cannot be proved from the known facts. We show that the indistinguishability relation has some desirable properties: it is reflexive, symmetric, and, while not transitive, it has a transitive “flavor.” The second formalization is an extension (modification) of the ordinary language equality by a similar default: two objects are equal if and only if their non-equality cannot be proved from the known facts. It appears to be less elegant from a formal point of view. In particular, it gives rise to multiple extensions. However, this extended equality is better suited for most of the applications discussed in this paper. (shrink)
